Staff memberAt Active Care Partnerships we provide care for people with a variety of needs.

From Acute Psychiatric Care which requires the service of a hospital to transitional care as a bridge to returning to the community. We provide residential care for adults with acquired brain injuries and specialist dementia care.

We work closely with the NHS trusts and Primary Care Trusts focusing on rehabilitative treatment programmes tailored to the needs of the individual.

What is common across all our facilities is the commitment of our staff to helping maintain the health and dignity of the people in our care. We want the time they spend with us to be as rewarding as possible.

Specialist Dementia Care

Service user doing some gardeningWe provide person centred care in a warm, secure and peaceful environment. Our services are equipped to ensure an environment which engenders a feeling of safety and security, engaging service users on all sensory levels.

Service users are involved in all aspects of social, recreational and therapeutic activities as well as life review work. All activities are structured to meet individual needs, abilities and preferences and are designed to maintain independence and promote skill development.

Dementia signposting is used to assist with orientation and service users are encouraged where possible in the development of their individualised plans or care. Support and involvement of family and friends is greatly valued.

Acquired Brain Injuries

Service user and staff doing some gardeningActive Care Partnerships has a specialist service that caters for adults who have an acquired brain injury. In order to provide the highest quality care and support possible for each individual, our dedicated specialist team include a neuropsychology team, physiotherapists, occupational therapists, speech and language therapists and a team of highly trained nursing and support staff.
