At Active Care Partnerships we provide a range of Supported Living facilities.
With this type of care individuals live in a house in the local community as tenants. They pay rent to the landlord or the owner of the house and have a set of keys to enter and leave the house as they wish.
The tenant has an agreement in place which covers their care support and has a nominated key worker who will be the primary support worker.
The house is the tenant’s home. They get to make the decisions and choose what happens in their own home. This includes the food they eat, when they get up and when they go to bed, what they buy, the activities they do and who comes to visit them in their own home.
The support we provide is dependant on the needs of the tenant. We respect the needs and wishes of the tenant whilst ensuring they remain healthy and safe. The role of the support worker is to help the tenant make decisions and live independently.
