Intermediate Care is available within our homes to provide rehabilitation for elderly individuals who are ready to leave hospital but are not ready to return to return to their own home.

Both physiotherapy and occupational therapy services are available in homes offering this service. A dedicated social worker manages the admission process right through to discharge from our home.

Facilities in the care home are dedicated to the clients receiving transitional care. A separate unit is usually provided away from people in the home on a permanent basis.

The length of stay for each person is usually based on their individual progress but there may be a maximum period of stay stated in the contract that would be negotiated by the care home and the local NHS Trust.

Beds are purchased by the local NHS Trust for and on behalf of the service user.
